FEE SCHEDULE - 2010
 

ASSESSMENT                           $100 (normally 1hr., 15 minutes)

Assessment includes a face to face interview and screening using instruments such as the SASSI and the AUDIT.  Assessments and referrals from other therapists or physicians are sufficient for entry into counseling or education at Evergreen.  By appointment.

EDUCATION                              $30 per 1 1/2 hour session; 4 sessions in series

Alcohol and drug education is designed to help people determine to what degree substance use has affected their lives.  It explains the differences between substance use, abuse and addiction, how to determine whether one has a problem or not, and what to do about the problem if there is one.  Meets Wednesdays  6:30-8:00 pm and Thursdays, 10:00-11:30am

INDIVIDUAL COUNSELING       $ 50 per session (1 hour)

      Individual Counseling is tailored to each client's needs.  It focuses on the specific issues which abuse of substances has caused in a person's life and facilitates the development of a plan to avoid further consequences as a result of substance abuse.  The number of sessions is determined by the extent of the substance abuse or dependence problem.   By appointment.

AFTERCARE                               $ 20 per session (1 hour)

Aftercare is for people who have completed a formal treatment program or have otherwise established sobriety and would like continuing support in a group facilitated by a therapist.  Fee is waived for those who completed counseling and education at Evergreen.  Meets Wednesdays, 5:00-6:00pm and Thursdays, 12:00-1:00pm.

SAP ASSESSMENT AND FOLLOW UP   $250 (for DOT Positive Drug or Alcohol Tests only)

SAP Assessment and follow up includes initial assessment and post treatment assessment - additional charges may be incurred if more intervention is required (e.g. additional positive drug tests.)
